Vendors should visit the Community Events page of the Health Department for you will find info here and the form here.
Vendors are to complete the Community Event Temporary Food Facility Application and submit to S.F.S. for approval.
Vendors must indicate the location of their food preparation site by providing their active Public Health Permit (or completing Sharing a Permitted Food Facility Agreement Form
A copy of the applicant’s government-issued identification must be attached to the application.
Vendors may submit applications to the Specialized Food Services Program via email to ehsfs@ph.lacounty.gov
